PAWS Incident Report Forms

A comprehensive safety program must have a reporting and tracking system for evaluation and improvement. The PAWS Incident Report Form is used for that purpose.

A student observing an unsafe act, an unsafe condition, or a near miss, is expected to immediately correct the situation and record it on a PAWS Incident Report Form (a PDF File). Additionally, safety suggestions or past-due maintenance items (leaky seal, unreadable or missing labeling, etc.) are reported on the same form.

Incident report drop-boxes are located in the Unit Operations Lab office, in the PSCC Control Room, and in the Bio-Processing Laboratory. The reports are collected and tabulated at the end of each experiment cycle, and each report is discussed at the next scheduled Safety Meeting.

The PAWS program includes an Incentive Program to encourage reporting of incidents.
